A parenthesis is located in the IHP template that provides language to “see additional forms per policy or procedure” to prevent duplicative documentation of school health service delivery. There are multiple interventions in the school health service delivery. The three types of intervention include:
Dependent Intervention: Dependent interventions are everything that parents provide to the school.
Example: Signed consent for a health service procedure, the supplies to perform the task, healthcare provider orders or a prescription from a health care provider.
Independent Intervention: Independent interventions are everything the school nurse provides while the student is at school
Example: The school nurse performs the nursing task with the supplies that the parents have provided by following the instructions from the healthcare provider (if applicable).
Collaborative Intervention: Collaborative interventions are everything the authorized or qualified personnel provide.
Example: The paraprofessional performs a student’s straight catheterization three times a day as delegated by the school nurse
A combination of all intervention types is called implementation of health services and interventions.
Example:
The student requires ongoing health services during the school year.
The school nurse may write in the box: “The student requires ongoing health services during the school year”.
