First Steps When Using the Template: Determining the Pages Needed

The school nurse must first calculate the number of pages needed for interventions to scale out outcomes or goals. Do not enter any information into the template until you have clicked the button on the bottom of page two for the number of pages needed. 

Please be aware that if the school nurse creates more than one goal, the nurse will also be responsible to evaluate each goal at a minimum annually. Nurses must evaluate the student’s intrinsic motivation, self efficacy, application, and mastery.
